Evergreen Endodontics ("we," "us," or "our") respects your privacy and is committed to protecting the information you share with us. This Privacy Policy explains how we handle information in two settings:
- Part 1 — HIPAA Notice of Privacy Practices describes how we use and disclose your protected health information (PHI) in connection with the dental and endodontic care we provide, as required by the federal Health Insurance Portability and Accountability Act (HIPAA).
- Part 2 — Website Privacy Policy describes how we handle information collected through this website and our other online channels.
This notice applies to Evergreen Endodontics and to all members of our workforce who may have access to your information. Effective date of this notice: October 17, 2026.
Part 1 — HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Pledge Regarding Your Health Information
We understand that information about your health is personal and sensitive. We are committed to protecting the privacy of your health information and to giving you notice of our legal duties and privacy practices.
We are required by law to:
- Maintain the privacy and security of your protected health information.
- Give you this notice explaining our legal duties and privacy practices with respect to your health information.
- Follow the terms of the notice currently in effect.
- Notify you if a breach occurs that may have compromised the privacy or security of your information.
We will not use or share your information other than as described here unless you tell us we may in writing. If you tell us we may, you may change your mind at any time by notifying us in writing.
How We May Use and Disclose Your Health Information
The following describes the different ways we may use and disclose your health information. For each category we explain what we mean and give examples. Not every use or disclosure in a category will be made, but all of the ways we are permitted to use and disclose information fall within one of these categories.
Treatment
We may use your health information to provide, coordinate, and manage your dental and endodontic care. For example, we may share information with your general dentist, your physician, or another specialist involved in your care, or we may send a prescription to your pharmacy. We may also contact you to provide appointment reminders or information about treatment alternatives or other health-related benefits and services that may be of interest to you.
Payment
We may use and disclose your health information to obtain payment for the treatment and services we provide. For example, we may share information with your dental insurance plan to verify coverage, obtain prior authorization, or submit a claim, and we may share information with a secondary carrier to coordinate benefits.
Health Care Operations
We may use and disclose your health information to run our practice and make sure that you and our other patients receive quality care. For example, we may use your information to review the quality and competence of our clinical staff, to conduct quality assessment and improvement activities, to train our staff, or to arrange for legal, accounting, or other business services.
As Required or Permitted by Law
We may use or disclose your health information when we are required or permitted to do so by federal, state, or local law. This includes, for example, disclosures to public health authorities to prevent or control disease or injury, to report suspected abuse, neglect, or domestic violence, to comply with health oversight activities, to respond to a court order or lawful subpoena, to assist law enforcement under limited circumstances, to prevent a serious and imminent threat to your health or safety or the health and safety of others, and to comply with workers' compensation laws. Washington law may provide additional protection for certain categories of information, such as mental health, sexually transmitted disease, HIV, and substance use disorder records; where state law is more protective, we follow state law.
Uses and Disclosures Requiring Your Written Authorization
We will obtain your written authorization before using or disclosing your health information for any purpose not described in this notice. This includes most uses and disclosures of psychotherapy notes, most uses and disclosures of health information for marketing purposes, and any sale of your health information. If you give us authorization, you may revoke it at any time in writing, and we will stop using or disclosing your information for the purpose covered by the authorization, except to the extent we have already taken action in reliance on it.
Fundraising and Marketing
We do not sell your health information. If we ever contact you for fundraising purposes, we will tell you how to opt out of receiving further fundraising communications.
Your Rights Regarding Your Health Information
You have the following rights with respect to the health information we maintain about you.
Right to Inspect and Copy
You have the right to inspect and obtain a copy of the health information that we maintain about you, including dental and medical records and billing records. To request a copy, submit a written request to our Privacy Officer. We will respond within the timeframe required by law and may charge a reasonable, cost-based fee for copies as permitted by law. In limited circumstances we may deny your request, and we will explain the reason in writing.
Right to Amend
If you believe that the health information we maintain about you is incorrect or incomplete, you may ask us to amend it. Your request must be in writing and must explain the reason for the amendment. We may deny your request if the information was not created by us, is not part of the information we keep, or is otherwise accurate and complete. We will respond in writing and, if we deny the request, explain how you can submit a statement of disagreement.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures of your health information that we have made. The list will not include disclosures for treatment, payment, or health care operations, or disclosures made pursuant to your authorization or certain other purposes permitted by law. Your request must be in writing and state the time period, which may not be longer than six years.
Right to Request Restrictions
You have the right to request that we restrict the health information we use or disclose about you for treatment, payment, or health care operations, or to someone involved in your care. We are not required to agree to a restriction, except that we must agree not to disclose your information to a health plan for payment or health care operations purposes if you paid for the service in full out of pocket and you request the restriction.
Right to Confidential Communications
You have the right to ask us to contact you in a specific way (for example, by cell phone or at a specific address) or to send mail to a different address. We will accommodate all reasonable requests and will not ask you the reason for your request.
Right to Choose Someone to Act for You
If you have given someone medical power of attorney, or if someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will confirm the person has this authority before we take any action.
Right to a Paper Copy of This Notice
You have the right to a paper copy of this notice at any time, even if you have agreed to receive it electronically. To obtain a paper copy, contact our office or our Privacy Officer.
